It looks like Pipedrive recently changed their JS asset host from pipedrivewebforms.com (whitelisted here) to webforms.pipedriveassets.com (per the script src in these instructions
https://support.pipedrive.com/hc/en-us/articles/208493165-Pipedrive-Web-Forms#section4).

I think the following rule would resolve the issue:

I neglected to include a bit of information last time -- adding it in case it helps someone test this change more quickly:

Affected site: http://www.woventeams.com
Behavior: We include a contact form at the bottom of our site that is powered by Pipedrive, a sales CRM tool, so that all leads automatically drop into our outbound sales queue. Because webforms.pipedriveassets.com is blocked per my previous post, the form only half-loads, then I see thousands of blocked requests in uBlock Origin.
Ad Blocker Information: uBlock Origin 1.14.22, no custom lists, Chrome 63.0.3239.132 64-bit
