Contact Forms on connectingdirectories.com not working

Some contact forms on my site not working.

Any of the contact forms that exist on a sub page of: "http://www.connectingdirectories.com/advertisers/" are not working. Disabled forms can be found here:

http://connectingdirectories.com/advert ... ontact-us/

Other forms on my site that are not subpages of "/advertisers/" are not disabled.
