E.g. http://andrewsullivan.theatlantic.com/t ... th-10.html - the whole page is blocked because they've wrapped all content in '<div id="ad_banner_top">'.
Strangely I couldn't find any other pages on the site that were affected.

Well, I guess sooner or later he will find out and will have to change it. Why would someone wrap their whole site in THAT id? It would be easier for him to change it.
Like you said, it is the only subdomain affected there. I will not remove a good element-hiding rule simply for one small blog (element-hides cannot be whitelisted either). You can either disable that element-hide rule or ABP for that one.
